Why do I need to reverify my service?
The Lifeline Administrator recently launched the National Verifier in your state. The National Verifier is an eligibility tool that allows the Program to confirm eligibility of enrolled subscribers in each state.
What if I recently recertified with Assist Wireless?
Regardless of whether have recently recertified with your wireless provider, you will need to complete the required forms and submit them to the Lifeline Administrator or you risk losing your Lifeline discount.
Will I still need to perform my annual recertification recertify with my Lifeline Provider this year?
If you have not yet recertified this year, you will not be required to recertify again with your wireless provider. (If you have already recertified with Assist Wireless this year, see FAQ#2).
What will I need to complete the form?
Most of the form is information about the household which receives the Lifeline discount.
You will need the following information to apply:
- Full legal name
- Date of birth
- Last 4 digits of your Social Security number (or Tribal identification number)
- Address (please see FAQ#8 for information regarding recent address changes)
You will also be asked to select your Lifeline Program qualifier (income or program). You will not be required to submit proof at this time, but may be required to provide additional documentation if the Lifeline administrator is unable to verify your eligibility within available government databases.
Can I send the completed form to my Wireless Provider?
No. Forms must be submitted directly to USAC from the Lifeline participant/subscriber: USAC Lifeline Support Center P.O. Box 7081 London, KY 40742. All other submissions will be denied.
What if I don’t have a printer?
If you do not have access to a printer, we recommend you try your local public library (ID may be required), or forward the form to a local print center, such as an Office Max, Staples, or UPS Store (call locations for specific directions on how to submit for printing services).

What address should I use if I have moved recently?
If you have moved since your last Lifeline eligibility recertification (or initial application, if with Assist for less than 1 year), you will need to make sure your address is updated with Assist Wireless. You can update your address here: [link] or call 855-392-7747.
Only one Lifeline discount is permitted per household, so failure to update your address could result in disconnection of your Lifeline service.
