Will I still need to perform my annual recertification recertify with my Lifeline Provider this year?

If you have not yet recertified this year, you will not be required to recertify again with your wireless provider. (If you have already recertified with Assist Wireless this year, see FAQ#2).

Most of the form is information about the household which receives the Lifeline discount.
You will need the following information to apply:

  • Full legal name

  • Last 4 digits of your Social Security number (or Tribal identification number)
  • Address (please see FAQ#8 for information regarding recent address changes)

You will also be asked to select your Lifeline Program qualifier (income or program). Can I send the completed form to my Wireless Provider?

No. Forms must be submitted directly to USAC from the Lifeline participant/subscriber: USAC Lifeline Support Center P.O. Box 7081 London, KY 40742. All other submissions will be denied.

What address should I use if I have moved recently?

If you have moved since your last Lifeline eligibility recertification (or initial application, if with Assist for less than 1 year), you will need to make sure your address is updated with Assist Wireless. You can update your address here: [link] or call 855-392-7747.

Only one Lifeline discount is permitted per household, so failure to update your address could result in disconnection of your Lifeline service.
