If you have service under an Assist Wireless Lifeline Phone Plan, we would like to say thank you for allowing us to serve you and inform you of the necessity of recertification.

Each year, you are required to recertify that you remain eligible to receive discounted phone service thru the Lifeline Phone Program. Through recertification your continued participation in one of the qualifying programs your service with us will be extended for an additional year. There will be no change or additional charges to your current account for recertifying.

Prior to the expiration date of your eligibility, you will receive a text, phone message, or a letter informing you that you must recertify your continued eligibility in the Lifeline Phone Program. If you fail to recertify within sixty (60) days after being notified, or if Assist is unable to verify your eligibility thru a state eligibility database, you will be deemed ineligible to continue participate the Lifeline Phone Program. Lifeline phone discounts you currently receive will be discontinued at that time and only Non-Lifeline discounted minute plans will be available for you to purchase.

If you are recertifying, please provide a photo copy of proof of participation. Examples of acceptable forms of proof include copies of your EBT food stamp card, Medicaid Card, a current award letter you have recently received in the mail, or a letter from your case worker. Please do not send original letters or actual ID cards, as they will not be returned to you. Your recertification may not be verified and completed without photocopies of proof provided to Assist Wireless in some states.

  • Online
    Simply click this link and follow the instructions to complete your recertification online. Please have scanned versions of your proof and ID ready to upload.
  • By Mail
    Mail your completed recertification form along with your proof of government assistance (if required) to: Assist Wireless at 2330 Gravel Dr., Ft. Worth TX 76118
  • By Fax
    Fax the completed recertification form along with your proof of government assistance (if required) to Toll Free 1-866-229-6433, Attention: Assist Wireless Recertification Department. Include your name, a contact number, and your cell number including area code.
