There is a $30 fee to replace your damaged or lost Lifeline cell phone. There is no fee to
replace a phone under warranty. You may replace your lost or damaged phone by mailing payment to the address listed below. Your handset will be mailed to you at the address you provide in the order. If you choose to mail a check or money order please send payment to the address below:
